This TeleCare service is provided by Phyathai Hospital Group. (hereinafter referred to as “Phayathai Hospital Group”, “us”, “us”, “our”), please read the terms and conditions of the TeleCare service (“Terms and Conditions”) carefully. You agree to be bound by these terms and conditions. including any practices related to these terms and conditions.
1. Service
1.1 TeleCare services (“Services”) provided through our website (“Websites”) or our applications (“Applications”) which consist of the following services:
(a) treatment and advice on general diseases or early diagnosis by a medical professional (hereinafter referred to as "medical") with telecommunication via video conferencing system through our website or application
(b) delivery of medicines as ordered by a doctor at your place of residence
(c) Clinic Connect service, which is a service to send a team to your home To collect samples for medical laboratory analysis about common diseases or early diagnosis
(d) Other services that Phayathai Hospital Group will provide additional services through the website. or application
1.2 The issuance of a medical certificate is at the sole discretion of the physician on a case-by-case basis.
2. Qualifications of service users
2.1 You must be over 20 years of age and have the ability to enter into legal acts. and abide by the terms and conditions under Thai law
2.2 We reserve the right not to provide the service if you are under 20 years of age.
3. Opening a service account
3.1 During registration to open a service account You will be required to provide us with various information, including your personal information, such as your name, ID number, contact number when you complete the registration process. 5.5 To use this service
(a) You will not use the Service for any unlawful purpose. and must comply with the laws or regulations including laws relating to intellectual property
(b) You must access the service for your own treatment or inquiry. You will not use another person's service account, whether or not authorized by the account owner. You must not impersonate another person. or any other act that misleads you as another person or is affiliated with any other person
5.5 We make no guarantees that this service will be available. or suitable for using the service outside Thailand
6. Additional conditions for drug delivery service at your residence
6.1 We will provide medication delivery service at your place of residence. according to the pre-agreed time and date If you are unable to receive your medication on the date and time Please notify us at least 24 hours in advance if you miss your appointment and cannot pick up your medication on the agreed date and time. You can pick up your medicine at the hospital of your choice from the moment you make your first appointment. Alternatively, you can schedule another drug delivery service. In each drug delivery You will be charged a service fee according to the specified rate.
6.2 Once you have received the medicine You will need to verify that the label is correct for the medicine prescribed by your doctor.
6.3 Please follow the instructions given on the medicine label. if you have an allergic reaction or have side effects from eating or using drugs or have questions about the medication received Please contact us without delay. When your doctor confirms your drug allergy We will replace the new medicine for you once you have returned the medicine to us.
6.4 We are not responsible for any damages. or defects in medicines delivered to you, unless you notify us orally or in writing. with respect to such damage or defect. and deliver the received medication in the same manner as you received it. along with the unopened or used packaging within the day following the date you received the medication. in that case We will take corrective action as follows: (a) ship you undamaged or defective medicines to you free of charge; or (b) refund the cost of any damaged medicines. or such defects
6.5 We will not be liable for any loss, damage or expense. that happened to you or other person for delays in drug delivery In the event that such delay is caused by you or you didn't pick up the pills, or the person at your place of residence refused to take the pills. or that your residential information is inaccurate or any other reason that It is not caused by our fault or events beyond our control. or our staff
7. Additional conditions for Clinic Connect service
7.1 We will provide Clinic Connect at your place of residence. according to the pre-agreed time and date If you are unable to receive the service on that date or time Please inform us at least 24 hours in advance if you miss your appointment or are not ready to receive the service on the agreed date and time. You can pick up the service at the hospital of your choice (eg Phyathai 3 Hospital) or you can schedule another Clinic Connect service. You will be charged a service fee according to the specified rate.
7.2 Please carefully follow the instructions for preparation before receiving this service.
7.3 Before the service Our staff will verify the name. and your personal information and may ask or ask you to show proof of identity. or any other information We will send you the test results according to the method you choose.
7.4 We will not be liable for any loss, damage or expense. that happened to you or other person for the delay in the analysis In the event that such delay is caused by you or you do not refuse to receive the service or you are not prepared for the service or any other reason that is your fault or events beyond our control. or our staff
8. Medical expenses and payment methods
8.1 To use this service You will be required to pay for services and expenses according to the items and rates that the network. Phyathai Hospital prescribed and displayed on the website.
8.2 You will be required to pay any service fees and expenses. After using the service through the video conferencing system. for treatment and/or advice including appointments for drug delivery or Clinic Connect services (if applicable)
8.3 You acknowledge and agree that We will not refund treatment and/or advice fees. This includes the drug delivery fee at your residence and Clinic Connect service fee in all cases, unless the conditions in clause 6.4 are met.
8.4 Your health insurance plan may or may not cover service fees and expenses. Therefore, please contact your agent or insurance company before using the service.
8.5 If the video conferencing system loses connection during the service and we cannot contact you to resume service within 24 hours. We reserve the right to charge you even if the service period is not complete.
9. Data Protection
9.1 We will comply with all steps and processes necessary to comply with the rules required by data protection laws. To maintain your personal and medical information and prevent the disclosure and use of your information. The transmission of information over the Internet can be insecure. We do not guarantee the security of personal information that you submit to us via the website or application. through the use of the Internet and you do so at your own risk in transmitting such information.
9.2 To use this service You authorize us to do the following.
